Gunshot wounds to the face present serious challenges to the facial reconstructive surgeon. These injuries result from assault, accident, or suicide attempts. In contrast to blunt facial trauma, ballistic injuries result in significant bone and soft tissue loss, the degree of which is not always apparent at initial presentation. Ballistics is the branch of science dealing with the natural laws governing projectile missiles and their predictable performances. Interior ballistics is the behaviour of missiles within the gun barrel; exterior ballistics is the study of missiles and their motion through the air; and terminal ballistics is the study of the penetration of media more dense than air by the projectile and of the interaction between the projectile and those media. The best possible function and aesthetics are achieved when debridement and reconstruction of hard and soft tissues are done at an early stage and the residual soft tissue deformities are addressed at a later stage with revision surgeries and local flaps. Extraoral wounds that cannot be closed primarily due to excessive tissue loss are best managed by employing local, regional pedicled flaps and microvascular flaps.
